Consent forms needed for Covid testing
25th February 2021
Dear Parents and Carers
Up to one-third of people who have Coronavirus experience no symptoms. Lateral flow testing is a quick, easy way to find out if someone has Coronavirus.
We are offering on-site Covid-19 testing to all our students and staff when they return to school during the week begining 8th March.
The test is voluntary, but I would strongly encourage everyone to take part.
Lateral Flow Testing: Lateral Flow Testing is quick and easy to do by using a swab from the nose and throat. The students perform their own test, this is then processed in school by trained staff. Results (which takes half an hour from testing) will be shared directly with the students participating. Where participants are under 16, parents or legal guardians will also be informed. Should a child test positive we will contact you directly to provide further advice and guidance. By testing in school prior to the return of students, we will help to stop the virus spread and help to keep our school open as safely as possible.
Once again - encourage everyone to take part!
Once the initial 3 tests have been completed (each being 3 to 5 days apart), participating students will then be given tests to complete at home twice a week; further information will be provided. Lateral Flow Testing is not a replacement for current prevention measures and it is essential that everyone continues to wash their hands regularly, use face coverings and maintain social distancing where required.
In order for your child to take part in the Lateral Flow Test programme it is essential that we have received a consent form. Paper copies of consent forms are being sent out to you - but you can also provide consent by completing this form here: https://forms.office.com/Pages/ResponsePage.aspx?id=fBnDD4eBT0q8Q1H7D7YOZFn1IxRwPkpAqYdawXwppSlUNFFaVDQ3QUhDQVNSVzg4SzM5Q001WjVHUy4u
Ideally, we want you to complete the online version of the consent form because it will enable us to register your child and upload results quickly and easily. Paper consent forms can be posted back to school or handed in to Reception.
More detailed information on the return of students to school will be with you soon. We are very much looking forward to our students returning.
Thank you for your support.
